All schools in Pennsylvania are required by state law to have completed health records on file for each student. Schools are randomly audited to determine compliance to these health laws and we need your cooperation to be compliant. Please email to haxe@plumsteadchristian.org, mail or drop off these forms to the Upper School BEFORE August 15th. Physical and/or dental exams can be completed on or after September 1, 2022.
Incoming 6th Graders: Physical Exam
Incoming 7th Graders: Dental Exam; Updated vaccines- 1 dose of Meningococcal Conjugate Vaccine (MCV) & Tetanus, Diphtheria, Acellular Pertussis Vaccine (Tdap-AFTER age 11) are required by 7th grade
Incoming 11th Graders: Physical Exam
Incoming 12th Graders: Updated vaccines- 2nd Meningococcal Conjugate Vaccine (MCV) vaccination-second dose is required at age 16 or prior to entry into 12th grade
